Submission of Phnom Penh VIA Forms and Deposit (Either 18 May (Mon CCA) or 20 May (Wed CCA)

Dear cadets interested in the Phnom Penh VIA overseas trip in December,

We will doing the collection of forms and deposit this coming week 9 during CCA. You can pass the items either on the 18 May (Mon) CCA or the 20 May (Wed CCA). These are the documents and deposit we are collecting if you are interested:

1. Form K Consent Form: Please remind your parents to strike off the allow/ does not allow words before they sign the form.
2. Form C for Singaporean students who require additional subsidy: Please attach both income statements from both your parents during submission. Subsidy is based on gross income, not take home income. If one of your parents are not working, you can ask your parents to attach a letter to say that your parent is a housewife and not working (and sign on the file).  FAS students do not need to attach the income statement. You only have submit the subsidy form and stick FAS on the checkbox.
3. Cheque deposit of $270 (for non FAS students) or $150 (for FAS students). The cheque should be addressed to Anderson Secondary School. Please refrain from paying in cash as it is a big amount to carry to school. You should also write your name and class at the back of the cheque. 
4. Photocopy of Passport with at least 6 months validity. If your passport is expiring, you can renew it and submit the passport at a later date.

Please do not consolidate the money and forms among your platoon before submission. Reason is because we need to individually check the documents and cheque to make sure they match. Collection will be made after CCA on 18 May and before CCA on 20 May. You only have to submit the forms and money if you are going for a trip. 

Please feel free to look for your teachers if you require any clarification or advice.
